Is breast reduction surgery safer than getting breast implants (to make them bigger)?

I know that women can develop health problems from getting surgery to make their breasts larger…Is it because they’re having silicone implanted into their body?

How is breast reductions surgery different? Is it safer? Is it possible for a woman to die from breast reduction surgery?

For breast augmentation, is it safer to go under the muscle or over the muscle?

My wife is considering breast augmentation, and currently we are in the research phase. We would both like to know which placement is generally safer, under or over the muscle. Please know that she has a small frame, teaches gymnastics, and is very active, we are not sure if any of that plays a factor but would like to get as much information as possible. Thank you.
